INTERDISCIPLINARY COLLABORATION Spotlight on Nutrition and Continuous Documentation Improvement Children are constantly growing and developing, which means that proper nutrition is extremely important. Poor nutrition, or malnutrition, may result in long-term health complications if left untreated. The most important step in treating malnutrition is identification and diagnosis by the medical provider. To support our providers in accurate and consistent recognition and diagnosis of pediatric malnutrition, an interdisciplinary team made up of registered dietitians, physicians and registered nurses created a malnutrition screening and diagnosis tool. This was made into a laminated card and is distributed to providers by our registered dietitians, who also explain how to use it. Additional support for accurate documentation of malnutrition is provided by clinical documentation specialists. This team approach to malnutrition, led by Lisa Ehle, BSN, RN corporate manager, CDI, is one more way we ensure each patient receives the highest quality care and our care team works together to bring their expertise to achieve the best outcomes.
conduct surveys. The second solution, Protenus, incorporates the Omnicell (the automated dispensing cabinets at all locations) with data from the EHR to monitor timely medication administration. Additionally, the Joint Commission often requests information on what the hospitals or sites are doing to reduce potential for diversion. Protenus provides a proactive solution rather than a reactive one. Transformational Leadership During the past two years, we have worked with the Ohio State Board of Pharmacy to have Dayton Children’s Pharmacy provide services for our new Shriners Children's Pharmacy in Ohio. Creating a pharmacy in a hospital (Shriners Children's) within a hospital (Dayton Children's) structure has never been requested previously in Ohio. Collaborating with the Board of Pharmacy to ensure all concerns were addressed and regulatory requirements were met was quite an achievement. Many hours were spent identifying key current needs and “What if ...” scenarios that would come up that have not occurred in the past. We anticipate that our Ohio pharmacy will not only meet standards, but rather, be leading the way in exceeding them.
governance approach to optimize nursing tools, functionality and documentation elements in our electronic health record. Optimization sessions are facilitated by our clinical transformation specialists at each location, which provides opportunities for local end-user engagement in decisions that affect their workflows related to the use of the electronic health record. Feedback is moved forward to a larger workgroup at the system level to ensure a consistent approach to change and timely adoption. This cyclical methodology allows for improved human-computer interactions allowing for streamlined workflows and improved end- user experience. Electronic Health Record Nursing Onboarding Training Project An interdisciplinary team was brought together to develop a plan for providing a consistent and improved electronic health record (EHR) onboarding training experience for nursing staff. This approach incorporates various training methodologies to prepare newly hired nurses in the efficient use of the EHR in the delivery of care. This revised process emphasizes different training tools and incorporates a hands-on opportunity, providing the user with a well-rounded experience in the use of the EHR. INNOVATION AND IMPROVEMENTS WITH PHARMACEUTICAL SERVICES Now in his fourth year, corporate director of pharmacy, Fernand Betancourt, PharmD, MBA, BCMAS, CPh, has been working diligently to advance pharmacy services system- wide. This year, two new solutions were implemented system-wide to address regulatory compliance. The first is called Compounding 360, which is focused on compliance within the pharmacy IV preparation room. This provides monitoring, tracking and reporting required data for review when State Boards of Pharmacies and The Joint Commission
