CERTIFICATE TO ACCOMPANY TEMPLE ASSESSMENT

I, , Potentate of _ ___________________________ Shriners, of _____________________, hereby certify that having first obtained approval of the Imperial Potentate issued pursuant to §332.7 of the bylaws of Shriners International, the annexed resolution relating to an assessment on its members for purposes general to the membership was adopted at a stated meeting of the Temple. I further certify that: (a) At least one weekʼs notice in writing was given to every member of the temple, stating the amount of the proposed assessment and the time and place when action would be taken. The notice was mailed on _ _____________________________________________ . A copy of the notice is attached hereto.
